A Case of Traumatic Occlusion of the Internal Carotid Artery in the Neck



Abstract
Since 1872, when the first case of a non-penetrating injury to the internal carotid artery in the craniocervical area was noted there have been 74 cases reported in the available literature.
We have experienced a case of the, traumatic.. occlusion of the internal carotid artery in the neck due to head injury.
A 35-year-old male was admitted to Kyung Hee Medical Center following an automobile accident in which he was rendered unconscious. Physical examination disclosed normal vital signs and there were no associated injuries. There were lacerated wounds on the left supraorbital area and abrasion of the left cheek.
Neurological examination revealed drowsy consciousness without cranial nerves involvement and all extremities moved well by noxious stimulus.
Simple skull X-ray showed linear fracture of the left frontal bone. Echoencephalogram-disclosed no evidence of midline structure shifting and electrocardiogram showed no abnormal findings.
A few hours later reexamination disclosed right hemiparesis without pupiliary change Left carotid arteriograrij showed a complete occlusion of the internal carotid-artery 2 cm distal portion from carotid bifurcation on neck. In the fifth hospital day right carotid angiogram showed collateral filling to opposite anterior cerebral artery and middle cerebral artery through anterior communicating artery. Patient¢¥s consciousness was gradually gained to alert state and noted dysphasia, and hemiparesis was unchanged.
